Listen for the whole story
Symptoms, routines, stress, sleep, family history, and goals all carry clinical meaning. Your plan begins with context—not a checklist.
Personalized · Preventive · Data-driven
Concierge medicine grounded in evidence-based medicine, whole-person care, and root cause analysis, bringing your story, screening, genetics, and wearable data into one thoughtful plan—so care can move beyond the standard snapshot.
A brief, no-pressure conversation to see if this approach fits your goals.
Whole-person care, guided by deeper context and designed around the life you want to keep living.
A wider lens
Good preventive care uses whole-person context and root cause analysis to ask not only “Is this normal?” but also “Is this optimal for you—and where is the pattern heading?”
Symptoms, routines, stress, sleep, family history, and goals all carry clinical meaning. Your plan begins with context—not a checklist.
Screening, biomarkers, genetics, imaging, and wearable trends can be more useful when interpreted together rather than in isolation.
Data matters when it leads to clear decisions. The aim is a practical, prioritized path you can understand and use.
Areas of focus
Whether you want to understand a symptom, reduce future risk, or make better use of health data you already have, the starting point is your concern—not a preset package.
What to expect
The process begins with fit and clarity. You’ll have room to explain what matters, understand the approach, and decide on a next step without pressure.

About the physician
Care philosophy
"I believe in treating the person as a whole. As William Osler pointed out, "Ask not what disease the person has, but rather, what person the disease has." I believe in knowing my members not only from the perspective of their medical issues but also their lifestyle, and, therefore, the influence on their health. Thus I'm able to empower them to make better choices and put them in charge of their own health." – Dr. Pooja Singh
Dr. Singh received her medical degree in India and completed her residency in New York City. Before joining Forward, she worked at Columbia University Medical Center, managing acute presentations of chronic diseases.
Dr. Singh has built a varied career around preventive primary care. She worked at Forward, a health technology startup that created the first CarePod, an experience that deepened her interest in how thoughtfully designed technology can make care more proactive, personal, and accessible.
Today, she brings that perspective to personalized medicine, combining evidence-based medicine, whole-person care, and root cause analysis with thoughtful use of screening, wearable trends, and genetic information. The goal is not more data for its own sake. It is clearer insight, earlier action, and a plan that feels relevant to your life.
